Our NHS Partners

NHS Derby and Derbyshire Integrated Care Board (ICB)

We are part of the NHS Derby and Derbyshire Integrated Care Board (ICB).

ICBs are NHS organisations that plan and fund local health services. They work with hospitals, GP practices, community services, local councils and voluntary groups to improve health and wellbeing.

Our ICB is part of Joined Up Care Derbyshire, which brings health and care partners together to:

  • Support better health for local people

  • Reduce inequalities in health outcomes

  • Make the best use of NHS resources

ARCH Primary Care Network (PCN)

We are a member of the ARCH Primary Care Network (PCN).

PCNs are groups of GP practices working together to provide more personalised and coordinated care. Through ARCH PCN, we offer patients access to additional services, such as:

  • Clinical pharmacists

  • Social prescribing link workers

  • First contact physiotherapists

  • Health and wellbeing coaches

PCNs help us to work more closely with community services, care homes, and other NHS teams to improve care in our local area.

Amber Valley Health

We are also part of Amber Valley Health.

Amber Valley Health delivers services on behalf of our PCN, such as extended access appointments and specialist clinics. You may sometimes see their name on services provided at or through our practice.
