We keep basic personal details (name, address, date of birth), medical history, test results, contact details, and notes from your visits.
Why we use your information
We use your data to provide you with healthcare, communicate with you (appointment reminders, test results), manage our services, and (where permitted) plan improvements or contribute to research.
How we protect your data
Your information is stored securely, on computers and/or in locked paper records. Only staff who need it to do their job can see it.
When we share your information
Sometimes we share your information with other health services involved in your care (for example, hospitals or community nurses).We only share when there is a lawful reason like your consent, or a legal requirement. We always take care to protect your confidentiality.
Your rights
You can ask us to:
- See a copy of your records
- Correct any mistakes
- Restrict how we use your information
- Ask us to delete it in certain cases

Opting Out of Data Sharing
National Data Opt-Out
You can choose whether your confidential patient information is used for research and planning across health and care services.

This choice is called the National Data Opt-Out.

It does not affect your individual care.

You can set your preference through the NHS App, the NHS website, or by calling 0300 303 5678.
Type 1 Opt-Out (Practice-based)
Some patients have registered a Type 1 opt-out (also called a Type 1 objection). This means their identifiable information is not shared outside the GP practice for research or planning.

Type 1 opt-outs are recorded here at the practice.

You can still ask us to apply or remove a Type 1 opt-out. To do this, please download and complete the Type 1 Opt-Out Form (PDF) and return it to reception.

This choice will not affect your care.
NHS England has said Type 1 opt-outs may be phased out in future, so the National Data Opt-Out is now the main way to register your preference.
