What we call for

We call for the following conditions annually, ususally around your month of birth;

  • Asthma
  • COPD
  • Hypertension
  • Atrial Fibrillation
  • Chronic Heart Disease
  • Stroke
  • Heart Failure
  • Mental Health
  • Dementia
  • Diabetes
  • Pre-Diabetes

 

You may be called for earlier or more frequently if required by a clinician or if you are taking any medication that needs blood tests for monitoring more regularly.

Our Invite Process

You will receive a letter just before your month of birth. This letter will contain your conditions and how to book.

If you have not booked or had your review appointment you will receive a follow-up text message or call in the middle of your month of birth.

 

It is important that you attend your annual review, to enable us to ensure that your condition is appropriately managed and controlled. If you are on any repeat medications these will also be reviewed and re-authorised during the same appointment as your annual review, it is therefore imperative that you attend this appointment. If not your medication may be stopped.

However, if you do not wish to attend and keep receiving reminders you can call reception on 01773 713201 and click option 5 to leave a message with your contact details, saying that you wish to decline for this year.

Booking Your Appointment

To book an appointment, please contact the Surgery on 01773 713201 and select option 5. You will be asked to leave a message containing your name, date of birth and contact telephone number.

You will be contacted by our reception team, within 72 working hours, to book you in for a review appointment.

If you have multiple conditions, where possible, we try and merge your appointments so you only have to attend for one review. Calling you back allows reception time to find suitable appointment slots for this as well as arranging any blood tests needed.

Depending on your condition, you may require a blood test before your review. If you need a blood test, the receptionist will inform you when they contact you to book your appointment.
