Fit Notes (previously Sick Notes)

We want to support you to stay healthy at work and return safely if you’ve been unwell.
This page explains when you need a fit note, how to request one, and where to find more guidance.
Our aim is to make the process simple and clear, so you can focus on your recovery.

For absences of 7 days or less

If you are off work for 7 calendar days or less, you do not need a fit note from a GP.


Your employer may ask you to complete a self-certification form (SC2). This is available from your employer or from the HMRC website.

For absences longer than 7 days

If you are off work for more than 7 calendar days, your employer can ask you to provide medical evidence.
This usually means a Statement of Fitness for Work (fit note).

What a fit note is

A fit note gives advice to help you get back to work safely.
Your GP may say that you are either:

  • Not fit for work

  • May be fit for work (with adjustments such as reduced hours or amended duties)

Since July 2022, fit notes may be issued by GPs, hospital doctors, nurses, occupational therapists, and some pharmacists.

Requesting a fit note

The quickest and easiest way to request a fit note is by using our online request form.

  • Use the form to request a new fit note or an extension.

  • Please make your request before your current fit note runs out.

  • In many cases, we may not need to see you in person.
